Grant Description
Alameda County COVID-19 Recovery and Resilience Project - Alameda County Health Care Services Agency, Public Health Department's Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved (the Initiative) will bring targeted mitigation services to African American, Latinx, and Pacific Islander populations, with a focus on serving those living in neighborhoods that are located in the Healthy Places Index low quartile (Q1 and Q2) zip codes.
Program strategies and activities implemented under the Initiative include:
Strategy 1: Expand existing and/or develop new mitigation and prevention resources and services to reduce COVID-19 related disparities among populations at higher risk and that are underserved.
Activity A: Expand community-based testing partnerships.
Activity B: Implement community coalition model: COVID-19 vaccine pods.
Strategy 2: Increase/improve data collection and reporting for populations experiencing a disproportionate burden of COVID-19 infection, severe illness, and death to guide the response to the COVID-19 pandemic.
Activity A: Expand community epidemiology and community evaluation capacity.
Strategy 3: Build, leverage, and expand infrastructure support for COVID-19 prevention and control among populations that are at higher risk and underserved.
Activity A: Implement community-based coaching and technical assistance.
Strategy 4: Mobilize partners and collaborators to advance health equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved.
Activity A: Implement community planning for recovery and resilience.
