Grant Description
Alameda County DTA - Alameda County's Overdose Data to Action Initiative (DTAI) represents a unique and innovative collaboration between HCSA, Alameda County Sheriff's Office, Coroner and Jail-Based Medical Examiner, community-based health safety-net organizations, and the University of California, Health Research for Action Center.
Through the DTAI opportunity, the collaborative will improve access and quality of SUD services for Black and Brown people and people experiencing homelessness in Alameda County by leading with data and expanding county-wide Substance Use Navigation (SUN) capacity and access to naloxone.
Alameda County DTAI leads will work with service providers across community, healthcare, and public safety settings and use their patient data to guide the deployment of SUN services where they are most needed.
In SUN and provider direct provision of SUD services, linkage to care and harm reduction efforts at these sites, patient data will be entered into health center EHRs. And through data sharing agreements, this data will be entered into the countywide Community Health Record (CHR) for use in coordination and linkages to care by systemwide providers.
This same data will flow into the countywide Social Health Information Exchange, where it will be analyzed by DTAI staff to inform evaluation and determine the outcome of harm reduction, linkages to care, and the impact of these activities on countywide fatal and non-fatal overdose rates.
