Grant Description
CDC-RFA-IP19-1901 Immunization and Vaccines for Children - The Federated States of Micronesia (FSM) is one of the six US affiliated island jurisdictions in the Pacific region, consisting of four island groups that are referred to as the Four States of the FSM. The island groups are Chuuk, Pohnpei, Kosrae, and Yap. Each state has its own unique settings, which include outlying neighboring islands that are only accessible by ships and most with limited or no connectivity at all.

FSM is one of the jurisdictions that have the primary responsibility for carrying out the public health assurance functions required to achieve the desired outcomes and performance goals established by CDC. The FSM NIP is transmitting its application to seek funding assistance from the US Center for Disease Control and Prevention (CDC) Immunization and Vaccine for Children funds to help continue its efforts to plan, develop, improve, and maintain its public health infrastructure. This will help assure high immunization coverage levels and low incidence of vaccine-preventable diseases by increasing its program activities to reach all children, adolescents, and adults within the nation.

Goal: To achieve and sustain high immunization coverage, reduce vaccination disparities and incidents of vaccine-preventable disease, improve readiness to respond to vaccine-preventable disease threats, and implement, develop, and/or enhance the FSM Immunization Information Systems (IIS) to support public health objectives.

The objective: Reduce, eliminate, or maintain elimination of cases of vaccine-preventable diseases; achieve and maintain effective vaccination coverage levels for universally recommended vaccines among young children. The FSM NIP, along with state programs, will implement all the activities developed under the IPOM 2019-2023 chapters.

Activity: Implement all developed SMART activities and performance measures that link to the new IPOM 2019-2023 on the 12 chapters. The FSM NIP will support the state programs in implementation, monitoring, assessment, trainings, and reporting of SMART activities to track progress toward the identified measures.

Evaluation: FSM NIP and state programs will participate in program technical assistance site visits/virtual technical support, CDC-led initiatives to assess and monitor activities, and submit mid-year progress reports and end-of-year/annual reports. The state programs are to submit annual work plans, monthly work plan reports, inventory reports and requests, hepatitis B monthly report, and update NIP during coordinators and program annual meetings.
