Grant Description
Fiscal Year 2025 expanded hours.

Operating under the Public Entity Model, Chambers County Public Hospital District #1 (CCPHD) and Chambers Community Health Center, Inc. (CCHC) provides all required primary, preventive, and supplemental medical health services, including oral health and mental health/substance abuse services, through three freestanding service delivery sites: Bayside Clinic, West Chambers Medical Center, and Dayton Medical Center.

In addition, CCHC operates two school-based health centers (Dayton ISD and Anahuac ISD) and a mobile unit.

The Public Entity organization, CCPHD, also operates Bayside Community Hospital and the Wellness Center at Bayside.

The current structure allows the leverage of various resources and sharing of services when appropriate and beneficial to the patient.

The organization's mission is to identify and serve the healthcare needs of the community, providing the best care possible, regardless of one's ability to pay.

CCHC’s primary service area includes Chambers County, southern Liberty County, and portions of East Harris County, with a target population consisting of identified service area residents with incomes below 200% of the federal poverty level.

CCHC fills a critical gap in healthcare access for the community and serves populations that might otherwise go without care.

Currently, CCHC is the only provider in Chambers County offering a sliding scale to communities outside of traditional operating hours (8 AM – 5 PM) at one service delivery site and desires to increase patient access by expanding hours of operation at two additional delivery sites.

Many low-income individuals work jobs with inflexible hours that coincide with traditional operating hours.

Expanded hours allow these individuals to access healthcare without risking job loss or lost wages.

Additionally, students will benefit from access to expanded hours which may lead to increased attendance, thereby allowing them more time in class to learn.

With more available hours for primary and preventive care, CCHC will improve overall community health outcomes through increased preventive care and earlier intervention in health issues, and patients may avoid unnecessary emergency room visits for non-urgent issues, leading to cost savings for both patients and the healthcare delivery system.

Through these visits, staff will identify high-risk patients and refer to community health workers to address social determinants of health, such as: food insecurity, housing insecurity, access to transportation, and other social determinants that may affect access to care, contribute to poor health outcomes, and exacerbate health disparities.

CCPHD proposes to utilize HRSA expanded hours funding (HRSA-25-084) to support an increase in personnel by 2 FTE primary medical care providers, 2 FTE medical assistants, and 0.25 FTE licensed professional counselor in order to increase operating hours at two service delivery sites, Bayside Clinic and Dayton Medical Center, by an additional 25 hours per week, allowing access to the health center’s services in the communities served outside of traditional operating hours.

By expanding operational hours, CCHC will significantly enhance healthcare accessibility for its underserved communities, leading to improved health outcomes, reduced healthcare disparities, and stronger, healthier communities.
