Grant Description
Health Center Program - Family Practice & Counseling Services Network (FPCSN or the Network) is submitting this Service Area Competition new (SAAT ID #154***; Philadelphia, Pennsylvania) application to continue services to residents of public housing and surrounding communities.

For the last 34 years, the Network was an FQHC under Resources for Human Development (previous grant no. H80CS00718).

That organization changed ownership and relinquished the grant on 6/30/25.

The Health Center has established the new FPCSN non-profit corporation, which meets all of the requirements of the HRSA Health Center Program, and is now applying as a new applicant for the area it has served for many years.

This application is being submitted to ensure that patients continue to have access to the vital services FPCSN provides.

FPCSN serves a large part of urban Philadelphia, where the residents are largely Black/African American (54.2%) and low income (49.6% under 200% of poverty).

The community needs assessment shows significant disparities in access to health care and the health of residents, compared to the state.

The target population has high levels of chronic illness, significant barriers to adequate health care, and a lack of culturally appropriate and accessible services.

Much of the service area is designated as medically underserved areas and as dental health professional shortage areas.

FPCSN collaborates with the Philadelphia Housing Authority and Department of Public Health and a number of local and state organizations, including the Pennsylvania Association of Community Health Centers.

The Network includes three full comprehensive service sites: Abbottsford/Falls, the Health Annex, and the Stephen and Sandra Sheller 11th Street Family Health Services and six school-based sites.

The Network’s sites have all been developed in collaboration with public housing residents who are the target of services.

FPCSN served 15,617 people in 2024, an increase from 15,214 in 2023 and 14,872 in 2022.

One FPCSN site has patient-centered medical home recognition.

FPCSN Health Center sites will continue to provide the full range of primary and preventive FQHSC services, including pre-natal, obstetric, and midwifery services; diabetes and nutrition education; podiatry, pharmacy; and case management.

FPCSN provides behavioral health services and has been a leader in the integration of behavioral health with primary care.

Medication for opioid use disorder services began in 2018.

A full range of oral health services is provided and accessible to all patients.

In recent years, in recognition of the trauma and stress experienced by patients and staff, the health centers have added complementary wellness services, including mind/body care such as yoga, acupuncture, and massage.

A longstanding continuous quality improvement system and a fully certified EHR assure quality services and efficiency.

The FPCSN board meets 330 requirements and is vested with full authority and responsibility for the operations of the FPCSN Health Center.

The board includes one member who is a representative of the Housing Authority and two others who are or have been public housing residents.

FPCSN is working to increase the number of patients served, but is now planning to serve at least 15,617 patients in the 2026 calendar year.
