Grant Description
River Valley Mobile Integrated Healthcare Network - The River Valley Mobile Integrated Healthcare Network (RV MIHN) proposes to implement a rural mobile integrated healthcare model to provide services for substance use disorders (SUD), specifically opioid use disorder (OUD), and behavioral health (BH) services.
The RV MIHN is led by the applicant, Osage Ambulance District (OAD). This project will serve a geographic catchment area of eleven zip codes spread through Gasconade, Maries, and Osage counties in central Missouri. The total population in the service area is 18,692 individuals and 36,632 for the three-county area overall.
RV MIHN healthcare services include SUD/OUD/BH, primary care, field initiation of buprenorphine, and referral to community-based treatment, recovery, and other supports to address health inequities and social determinants of health. Partners include area certified community behavioral health clinic, Compass Health Network, and federally qualified health center, Community Health Center of Central Missouri Osage County Health Department, and the Osage County Anti-Drug Community Action Team.
Whole person care will be delivered through the evidence-based mobile integrated healthcare model. The goals of the project are to improve referrals to appropriate treatment and recovery supports for high-risk individuals, increase local workforce capacity to engage, treat and support recovery of individuals with SUD/OUD and/or behavioral health needs, develop a replicable and sustainable MIH payment model supported locally and by Medicaid and other payers, and meet all grant deliverables on time and within budget.
The objectives are to identify and enroll up to 450 individuals in the RV MIHN program and provide SUD/OUD training and education to 2,000 individuals over the four-year period.
Goal 1: Implement the mobile integrated healthcare model that supports first responders and other key community sectors to administer a drug/device approved for emergency reversal of an opioid overdose.
Objectives:
1A. By 12/31/23, launch the RV MIHN that is supplied with drugs or devices approved for emergency reversal of known or suspected opioid overdose. The RV MIHN will provide emergency care, follow-up telehealth appointment for medications for opioid use disorder (MOUD), and other primary care, in the patient's home.
1B. The RV MIHN will train first responders and key community sectors on carrying and administering a drug/device for emergency treatment of an overdose by hiring an educator.
Goal 2: Expand access to SUD/OUD/BH services by raising community awareness of services, implementing outreach strategies, and building referral networks.
Objectives:
2A. Increase the awareness of RV MIHN and its available services through the development and implementation of outreach/engagement strategies by hiring a project director (PD), recovery support specialist (RSS), and community health worker (CHW).
2B. Provide comprehensive support services to all patients enrolled in RV MIHN.
2C. Complete assessments for every patient to provide warm handoff referrals to treatment, recovery, and other support resources as identified.
2D. Increase regional awareness of the availability of RV MIHN services and access points through targeted media messaging to reach a minimum of 20,000 views.

UNDER THESE SECTIONS, CSAT, CMHS AND CSAP SEEK TO EXPAND THE AVAILABILITY OF EFFECTIVE SUBSTANCE ABUSE TREATMENT AND RECOVERY SERVICES AVAILABLE TO AMERICANS TO IMPROVE THE LIVES OF THOSE AFFECTED BY ALCOHOL AND DRUG ADDITIONS, AND TO REDUCE THE IMPACT OF ALCOHOL AND DRUG ABUSE ON INDIVIDUALS, FAMILIES, COMMUNITIES AND SOCIETIES AND TO ADDRESS PRIORITY MENTAL HEALTH NEEDS OF REGIONAL AND NATIONAL SIGNIFICANCE AND ASSIST CHILDREN IN DEALING WITH VIOLENCE AND TRAUMATIC EVENTS THROUGH BY FUNDING GRANT AND COOPERATIVE AGREEMENT PROJECTS.
