Overview
Grant Description
Healing Families & Communities Project - Under the proposed Healing Families & Communities Project (HFCP), we will expand comprehensive treatment, early intervention, and recovery support services for transitional aged youth (ages 18-25) with substance use disorders (SUD) and/or co-occurring substance use and mental disorders (COD), and their families/primary caregivers.
Our primary catchment areas of focus include East Harlem and the South Bronx, in New York City; however, we understand that the needs of justice-impacted New Yorkers are great, and will extend outreach beyond the outlined catchment area into other Harlem, Bronx, Brooklyn, and Queens neighborhoods as well.
We will provide 450 unduplicated justice-impacted transitional aged youth and their families (over 5 years) with evidence-based clinical treatment services paired with recovery supports. HFCP participants will engage in 6-months of clinical treatment, where they will participate in an individual case management session, as well as at least 2 evidence-based/informed group sessions per week.
Enrollment in groups such as Seeking Safety, Anger Management, Conflict Resolution, Healthy Coping Skills, Relapse Prevention, and/or Healthy Relationships will be based on individualized needs and can change throughout the course of treatment (depending on need). Participants will also receive toxicology screenings on a regular basis.
We will also host quarterly Family & Youth Community Working Group meetings with our partners identified in Attachment 5. The goal of the group is to ensure early intervention and recovery support services are available to meet the complex needs of transitional aged youth and their families in recovery. The group can share information and work collectively to address policy issues and advocate for the de-stigmatization of addiction, recovery, and incarceration.

SAMHSA WAS GIVEN THE AUTHORITY TO ADDRESS PRIORITY SUBSTANCE ABUSE TREATMENT, PREVENTION AND MENTAL HEALTH NEEDS OF REGIONAL AND NATIONAL SIGNIFICANCE THROUGH ASSISTANCE (GRANTS AND COOPERATIVE AGREEMENTS) TO STATES, POLITICAL SUBDIVISIONS OF STATES, INDIAN TRIBES AND TRIBAL ORGANIZATIONS, AND OTHER PUBLIC OR NONPROFIT PRIVATE ENTITIES. UNDER THESE SECTIONS, CSAT, CMHS AND CSAP SEEK TO EXPAND THE AVAILABILITY OF EFFECTIVE SUBSTANCE ABUSE TREATMENT AND RECOVERY SERVICES AVAILABLE TO AMERICANS TO IMPROVE THE LIVES OF THOSE AFFECTED BY ALCOHOL AND DRUG ADDITIONS, AND TO REDUCE THE IMPACT OF ALCOHOL AND DRUG ABUSE ON INDIVIDUALS, FAMILIES, COMMUNITIES AND SOCIETIES AND TO ADDRESS PRIORITY MENTAL HEALTH NEEDS OF REGIONAL AND NATIONAL SIGNIFICANCE AND ASSIST CHILDREN IN DEALING WITH VIOLENCE AND TRAUMATIC EVENTS THROUGH BY FUNDING GRANT AND COOPERATIVE AGREEMENT PROJECTS. GRANTS AND COOPERATIVE AGREEMENTS MAY BE FOR (1) KNOWLEDGE AND DEVELOPMENT AND APPLICATION PROJECTS FOR TREATMENT AND REHABILITATION AND THE CONDUCT OR SUPPORT OF EVALUATIONS OF SUCH PROJECTS, (2) TRAINING AND TECHNICAL ASSISTANCE, (3) TARGETED CAPACITY RESPONSE PROGRAMS (4) SYSTEMS CHANGE GRANTS INCLUDING STATEWIDE FAMILY NETWORK GRANTS AND CLIENT-ORIENTED AND CONSUMER RUN SELF-HELP ACTIVITIES AND (5) PROGRAMS TO FOSTER HEALTH AND DEVELOPMENT OF CHILDREN, (6) COORDINATION AND INTEGRATION OF PRIMARY CARE SERVICES INTO PUBLICLY-FUNDED COMMUNITY MENTAL HEALTH CENTERS AND OTHER COMMUNITY-BASED BEHAVIORAL HEALTH SETTINGS
