Grant Description
Northeast Texas CCBHC Network - Community HealthCore will improve and advance its existing delivery of CCBHC services in nine largely rural North East Texas counties. Planned resource enhancements include addressing health care access disparities experienced by youth and veterans, improving care coordination for all consumers, and introducing measurement-based care to enhance treatment outcomes.

THE PURPOSE OF THIS PROGRAM IS TO INCREASE ACCESS TO AND IMPROVE THE QUALITY OF COMMUNITY MENTAL AND SUBSTANCE USE DISORDER TREATMENT SERVICES THROUGH THE EXPANSION OF CCBHCS. CCBHCS PROVIDE PERSON- AND FAMILY-CENTERED INTEGRATED SERVICES.
