Grant Description
CMHC Route 180 Grant will target high utilizers of crisis services and seek to engage them in needed supports through outpatient services, in an effort to reduce re-hospitalization. With SAMHSA CMHC funds, Red Rock Behavioral Health Services (RR) will launch Route 180, a project designed to address gaps in clinical services for those most impacted by COVID – adults with serious mental illness (SMI) and co-occurring disorders (COD).
The catchment area for the proposed project is RR's 14-county service area in central, rural western, and eastern Oklahoma (OK). RR will target high utilizers of crisis services, including those on the state's Most in Need (MIN) list and those discharged from inpatient psychiatric treatment and crisis units, providing a mobile outreach and treatment model delivered from our Oklahoma City and Weatherford Behavioral Health Clinics.
Among the 3,986 individuals treated by the crisis stabilization units within RR's catchment area, approximately 50% (1,669) left the crisis unit without receiving any follow-up services, in part because of the lack of access to immediate outpatient supports and the current extensive intake process to receive outpatient care. Furthermore, given the increasing needs among consumers, RR must address the needs of staff members who experience vicarious trauma; many employees have also experienced personal and economic hardship due to the pandemic.
Through Route 180, RR will deliver clinical services to 400 unduplicated individuals each year, for a total of 800 individuals served. Goals include reducing high-risk repeated crisis unit stays for adults by establishing two mobile treatment teams known as Bridge Teams (BT) to expand RR's outpatient behavioral health continuum capacity to 200 adults. Program clients will demonstrate clinical improvement associated with their individual mental health and substance use disorder (SUD) needs. Additionally, RR aims to reduce stress and secondary trauma among RR staff through participation in wellness activities.
RR will implement all CMHC required services, including sustaining its telehealth infrastructure; providing outpatient mental health services through the BTs; providing trauma-informed screening, assessment, diagnosis, and patient-centered treatment planning and treatment delivery; providing clinical and recovery support services through case management and peers; and addressing the mental health needs of staff through a Red Rock Cares program and access to an Employee Assistance Program. RR will also provide staff training on disparities, including cultural and linguistic competence.
Route 180 will integrate a range of evidence-based programs, including motivational interviewing, cognitive-behavioral therapy, TEAM (Testing-Empathy-Agenda Setting-Methods) CBT, Components for Enhancing Clinical Experience and Reducing Trauma (CE-CERT), Collaborative Assessment and Management of Suicidality (CAMS), Integrated Dual Disorder Treatment, Medication-Assisted Treatment, Assertive Community Treatment, and Applied Suicide Intervention Skills Training.
RR is well-positioned to implement Route 180, given its status as a certified CMHC by the state of OK and a certified community behavioral health center, along with its 40 years of experience providing services to individuals with the most complex needs. RR will partner with SAE and Associates Behavioral Health Evaluation to lead the evaluation efforts.
