Grant Description
Hennepin County Mental Health Center Quality Improvement Project to Certify as a Community Behavioral Health Clinic. The MHC will improve quality of services, impacting all persons served and specifically targeting the outcomes of the homeless and housing insecure population through improved integration with human services and primary care. The MHC will also increase data analysis capacity and sharing of information to expand partners' abilities to report on costs and coordination of care outcomes.
