Grant Description
Oneida Nation Special Diabetes Program for Indians - Our purpose is to empower individuals with diabetes to make healthy decisions and learn how to manage their disease to be as healthy as possible. Through clinical outcome data, program evaluation, and interdisciplinary health care provider input, we have developed programs and services to meet the needs of the community.

Our goal is to educate people living with diabetes. By giving them the skills necessary to control their disease, we hope patients can prevent future complications. The diabetes team works in coordination with the patients' primary care provider. We offer programs to increase physical activity, opportunities to be active and nutritional education, and a behavioral health specialist.

We work in collaboration with other departments such as: Health Promotions, Behavioral Health, Oneida Family Fitness Center, our WIC department, Dental, Optical, and Pharmacy. The proposed project will provide funding so that we can continue to offer our programs and improve and maintain the health of our community members living with diabetes.

Our program aims to improve the patient experience of care and education, to improve the health of individuals and our community, and provide the most up-to-date care. Funding allows for diabetes team members to stay up-to-date on their practice and licensure by allowing them to attend local and national diabetes conferences to deliver the highest quality of care.
