Crow Service Unit SDPI: Good Medicine - Baa Laa Wii Lalaa Itchaa Special Diabetes Program for Indians Diabetes Prevention and Treatment Program and Community Outreach
The Crow Service Unit SDPI: Good Medicine - Baa Laa Wii Lalaa Itchaa program is focused on strengthening our comprehensive diabetes prevention/treatment program. This program incorporates proven diabetes support, education, treatment, and prevention methods. We utilize a patient-centered, whole person, team-based approach to "wrap" services around our participants by meeting them where they are at.
The program initially began with clinical practice and has since expanded through current and developing community partnerships. These partnerships aim to engage, enable, and empower program participants and community members to improve their overall health and well-being.
By creating an integrated, patient-centered diabetes program, we promote primary (prediabetes prevention), secondary (diabetes prevention), and tertiary (diabetes complications prevention/slowing disease process) intervention measures. Simultaneously, we focus on building individual and community resiliency to overcome barriers of care. This approach has the potential to incite culture change and raise the standards for health equity for IHS beneficiaries.
