APPLICATION PURPOSE: THIS APPLICATION IS FOR FY2022 SECTION 5310 FUNDING APPORTIONED TO THE STATE OF GEORGIA (SMALL URBAN AND RURAL) AS WELL AS THE ATLANTA AUGUSTA COLUMBUS AND SAVANNAH UZAS IN THE AMOUNTS LISTED BELOW: - ATLANTA URBANIZED AREA 200K+: $1913852- COLUMBUS URBANIZED AREA 200K+: $139156- AUGUSTA-RICHMOND URBANIZED AREA 200K+: $212900- SAVANNAH URBANIZED ARE 200K+: $140654- GA URBANIZED 50K-199K: $886223- GA NON-URBANIZED LESS 50K: $1158104TOTAL: FEDERAL $4450889STATE SHARE $1001450.03TOTAL ELIGIBLE COST $5452339.03CAPITAL PUBLIC TRANSPORTATION PROJECTS PLANNED DESIGNED AND CARRIED OUT TO MEET THE NEEDS OF SENIORS AND INDIVIDUALS WITH DISABILITIES = $4450889MINIMUM 55% WILL BE USED TO PROVIDE SERVICES TO THE ELDERLY AND DISABLED INDIVIDUALS WITH 2447988.95 BEING USED TO PROVIDE TRANSPORTATION SERVICES THROUGH GADHS COORDINATED TRANSPORTATION SYSTEM; $450000 USED FOR TRIP$ AND SUB RECIPIENT ARC PROJECTS; AND ADMINISTRATIVE PURPOSES = $445088.90.; ACTIVITIES PERFORMED: PROVIDE TRANSPORTATION SERVICES FOR THE ELDERLY AND DISABLED WITHIN 12 ASSIGNED GA DHS REGIONS.; EXPECTED OUTCOMES: IMPROVE QUALITY OF LIFE FOR ELDERLY AND DISABLED CONSUMERS BY PROVIDING NON-EMERGENCY TRANSPORTATION TO AND FROM REQUESTED LOCATIONS I.E. WORK SENIOR CENTERS MEDICAL APPOINTMENTS ETC.; INTENDED BENEFICIARIES: ELDERLY AND DISABLED CONSUMERS WITHIN GEORGIA; SUBRECIPIENT ACTIVITIES: 19 TOTAL SUBRECIPIENTS - 7 GOVERNMENT ENTITIES 2 PRIVATE NON-PROFIT PROVIDERS 1 PRIVATE FOR PROFIT PROVIDER 8 REGIONAL COMMISSIONS AND ATLANTA MPO DESIGNEE ATLANTA REGIONAL COMMISSION (ARC)
