Grant Description
Advancing Health Literacy with Community Health Workers - Health Literacy in Context of the COVID19 Vaccine is an Urgent Challenge. The City of Reading, located in Berks County, Pennsylvania has a Robust Minority Population with 67% of the Residents Identifying as Hispanic or Latino. With Only Half of Eligible Hispanics Having Received at Least One Dose of the COVID-19 Vaccine, the City is at High Risk for Experiencing Continued Health Disparities in COVID-19 Outcomes. The City of Reading Proposes a Comprehensive and Collaborative Plan to Ensure that Our Citizens have the Capacity to Obtain, Process, and Understand Basic Health Information and Services Needed to Make Appropriate Health Decisions when Related to COVID-19 and to Advance the Health People 2030 Objectives. This Effort will be Accomplished in Partnership with the Reading Area Community College, a Minority Serving Institution, Penn State College of Medicine?s CDC-Funded Racial and Ethnic Approaches to Community Health Program, and Latino Connection, a Minority-Owned Business. We will Create an Effective Approach to Improving Health Literacy by Creating a Community-Centric, Systems-Focused Peer Reviewed Program that can be Effortlessly Integrated into Health Systems Already Fatigued due to the Pandemic. The Proposed Program will Consist of Three Main Components: (1) Training Community Health Workers to Conduct Health Literacy Assessments of Local Health Systems, (2) Providing Reports of Health Literacy Environments and Supportive Resources to Improve to Health Literacy within Health Care and Community Organizations, and (3) Conducting Rapid-Cycle Quality Improvement Initiatives of Programmatic Efforts using the PDSA (Plan-Do-Study-Act) Cycle.
