Grant Description
This application is to request FY2022 Section 5310 funds apportioned to Phoenix-Mesa UZA in the total amount of $5,178,379. There is $2,122,795 of local match coming from donations, fundraisings, and dedicated sales tax from the Regional Public Transportation Tax or City Sales Tax.

Purpose:
The purpose of the award is to provide funding for the purchase of replacement and expansion vehicles, mobility management, preventive maintenance, dual cameras for vehicles, and operating assistance.
Activities to be Performed:
Activities to be performed are the purchase of replacement and expansion vehicles, preventive maintenance, mobility management, dual cameras for vehicles, and operating assistance.
Expected Outcomes:
The expected outcomes are the purchase of the expansion and replacement vehicles, preventive maintenance, mobility managers' salaries who coordinate for the region, and operating assistance for programs that are for the intended beneficiaries.
Intended Beneficiaries:
Elderly persons with disabilities.
Subrecipient Activities:
The designated recipient City of Phoenix will subaward funds to the following subrecipients: ACCEL, Benevilla, Beatitudes Foothills, Caring Corp, Gompers Civitan, Chandler Gilbert ARC, The Centers for Habilitation (TCH), Northwest Valley Connect, Stand Together and Recover Centers (S.T.A.R.), Scottsdale Training and Rehabilitation Services (STARS), The Opportunity Tree, ValleyLife, Gompers One Step Beyond, Southern Arizona Association for the Visually Impaired (SAAVI), Valley Metro/RPTA, Arizona Center for the Blind and Visually Impaired, Hacienda Healthcare, Marc Community Resources, Valley Metro/RPTA, Valley of the Sun YMCA, and Scottsdale Training and Rehabilitation Services (STARS).
The selected projects for the Section 5310 program being applied for in this grant application are included in the locally developed Coordinated Public Transit-Human Services Transportation Plan. The current plan, MAG Human Services Coordinated Transportation Plan, was revised in January 2022. Link: https://azmag.gov.
