The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $20,000,000 for Year 1, subject to the availability of funds. Through direct service delivery support in CDC districts, the recipient(s) will improve access and uptake of HIV services to address the treatment gap and sustain epidemic control, in line with the strategic direction and goals of the PEPFAR Country Operational Plan. Interventions will be tailored to site- and population- specific needs across the HIV care continuum with a focus on retaining patients on treatment, using efficient, client-centered approaches including differentiated service delivery models, case management approaches and risk stratification tools to identify those patients who are more likely to have sub-optimal treatment outcomes. Innovative service delivery models will be implemented to support the provision of optimized antiretroviral treatment and retention in care, including increasing access to advanced HIV services, approaches to decongesting high volume clinics and improving the quality of clinical services. Routine data collection and analyses at site level will guide program improvements and the identification of barriers to care. The recipients will also facilitate a gradual transition of direct service delivery to the local district health systems for long term sustainability. In the era of the COVID-19 pandemic, additional site level support will be provided to ensure infection prevention and control standards, screening, testing and treatment of clients.
