The purpose of the State Offices of Rural Health Coordination and Development Program (SORHCDP) is to develop and maintain activities that will build the capacity of the 50 SORH and rural stakeholders nationwide by strengthening leadership, increasing opportunities for collaboration, offering education on policy and emerging health issues, and supporting the core functions of the SORH. This in turn will improve health care in rural areas.

Background
The State Offices of Rural Health Coordination and Development Program (SORHCDP) aims to enhance the capacity of the 50 State Offices of Rural Health (SORH) and rural stakeholders across the United States. This program, federally funded since 1991, focuses on strengthening leadership, fostering collaboration, providing education on policy and emerging health issues, and supporting the core functions of SORH to improve healthcare in rural areas.

The initiative is designed to address the unique challenges faced by rural communities and ensure better access to quality healthcare for nearly 60 million people living in these areas.

Grant Details
The SORHCDP will support activities that include:

1) Leadership Development: Providing leadership development at national, regional, state, and local levels to increase the impact of SORH.

2) Partnerships and Collaboration: Facilitating partnerships and collaboration to improve data exchange and best practices supporting rural health.

3) Addressing Emerging Rural Health Issues: Assisting in coordinating rural healthcare through education on policies and emerging issues.

4) SORH Capacity and Efficiency: Enhancing capacity building for SORH core functions to improve efficiency.

Applicants must collaborate with all 50 SORHs and rural stakeholders nationwide.

Eligibility Requirements
Eligible organizations include non-profits (with or without a 501(c)(3) status), for-profit organizations (including small businesses), state, county, city, township, special district governments (including the District of Columbia), Native American tribal governments, and Native American tribal organizations. Individuals are not eligible to apply. Applications must target all 50 State Offices of Rural Health.

Period of Performance
The period of performance is set for five years, from August 1, 2025, to July 31, 2030.

Grant Value
The total expected funding available for FY 2025 is $950,000 with a maximum funding range per award up to $950,000 per budget period.

Place of Performance
The grant will be performed nationwide across all states in the U.S., including the District of Columbia and U.S. territories.
