Background
The Service Area Competition (SAC) is a funding opportunity announced by the U.S. Department of Health and Human Services under the Health Center Program. The purpose of this funding is to ensure continuity of affordable, accessible, and high-quality primary health care in communities currently served by the Health Center Program.

The program aims to improve the health of underserved populations, including individuals experiencing homelessness, migratory and seasonal agricultural workers, and residents of public housing.

Grant Details
The SAC allows organizations to compete for funding to provide comprehensive primary health care services in designated service areas. Applicants must propose to serve at least 75% of the Patient Target listed in the Service Area Announcement Table (SAAT) for calendar year 2026.

Application types include Competing Continuation for current award recipients, Competing Supplement for existing recipients adding new sites, and New applications from organizations not currently funded. Each service area has a unique Service Area Identification Number (SAC ID) that must be included in applications.

The total anticipated available funding is approximately $244,461,600 for up to 88 awards over a performance period from May 1, 2025 through April 30, 2028.

Eligibility Requirements
Eligible applicants include private non-profit entities or public agencies in the U.S. or its territories. Tribal and community-based organizations are also eligible. Applicants must provide all required health services without regard for patients' ability to pay and must demonstrate a substantive role in the project.

Period of Performance
The period of performance for awarded grants will be from May 1, 2025 through April 30, 2028, allowing for up to three years of funding.

The total value of the grant opportunity is approximately $244,461,600 with an estimated number of awards up to 88. Individual award amounts may vary based on the proposed budget and patient targets.

Place of Performance
The grant will be performed in specific geographic locations as identified in the Service Area Announcement Table (SAAT), which includes various service areas across the United States.
