Background
The Rural Health Care Services Outreach Program is administered by the Health Resources and Services Administration (HRSA) under the Federal Office of Rural Health Policy. This community-based grant program aims to support organizations in promoting rural health care services outreach by improving and expanding the delivery of health care services to include new and enhanced services in rural areas.

The program's goal is to strengthen local resources and capacity in rural communities through collaborative consortiums that include three or more health care providers. This enables communities to develop innovative approaches to address their unique challenges and factors contributing to rural health disparities.

Grant Details
The program aims to expand the delivery of health care services exclusively in rural communities, focusing on outreach and education activities targeting unique health needs. Award recipients will design and implement projects tailored to meet the specific health needs of rural populations, including maternal health as a new focus area.

Successful applicants must demonstrate experience serving or capacity to serve rural underserved populations, specify a target rural service area, and represent a consortium of at least three healthcare provider organizations. Projects should utilize evidence-based or innovative models for service delivery and involve community engagement throughout the project duration.

Eligibility Requirements
Eligible applicants include all domestic public or private non-profit and for-profit entities, such as 501(c)(3) non-profits, public and private institutions of higher education, state and county health departments, hospitals (including rural emergency hospitals), Native American tribal governments, community-based organizations, federally qualified health centers, and rural health clinics.

Individuals are not eligible. Applicants must have demonstrated experience serving rural underserved populations and include only HRSA-designated rural areas as target service areas.

Period of Performance
The period of performance for awarded projects will be from May 1, 2025, to April 30, 2029, with funding provided in 12-month budget periods over a total four-year period.

Grant Value
The total expected funding available for FY 2025 is $13 million. Up to 50 grants will be awarded: up to 40 Regular Track awards with funding up to $250,000 per year per project and up to 10 Special Track awards with funding up to $300,000 per year per project.

Place of Performance
The grant activities will be performed in HRSA-designated rural areas across the United States.

Other eligible entities may include but are not limited to: Public and private institutions of higher education State and county health departments Hospitals, including rural emergency hospitals Native American tribal government and organizations Community-based organization Federally qualified health centers Rural health clinics Tribal exception: HRSA is aware that tribes and tribal organizations may not meet the Employer Identification Number (EIN) or Unique Entity Identifier (UEI) requirement of this NOFO. As a result, tribes and tribal organizations that only have one EIN or UEI or are unable to demonstrate that the network is composed of at least three unique entities, may request a tribal exception. Applicants must request a tribal EIN exception in Attachment 14: Tribal EIN exception request. * Domestic means the 50 states, the District of Columbia, the Commonwealth of Puerto Rico, the Northern Mariana Islands, American Samoa, Guam, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau. Individuals are not eligible applicants under this NOFO.
