The purpose of this Notice of Funding Opportunity (NOFO) is to build evidence about the characteristics and value of primary care that influence patient outcomes and advance health equity, such as care coordination, continuity of care, and comprehensiveness of care, person-centered, whole healthcare, and trust, and how these can be improved and effectively delivered to strengthen primary healthcare.
Background
The Agency for Healthcare Research and Quality (AHRQ) aims to produce evidence to make healthcare safer, higher quality, and more accessible, equitable, and affordable. Primary care is a core component of the United States healthcare system, and a strong primary care foundation is necessary to the health of people and populations. The purpose of this Notice of Funding Opportunity (NOFO) is to build evidence about the characteristics and value of primary care that influence patient outcomes and advance health equity, such as care coordination, continuity of care, and comprehensiveness of care.

Person-centered, whole healthcare, and trust are also important aspects to be considered.

Grant Details
The NOFO will prioritize research projects that aim to improve primary care in terms of quality, safety, access, affordability, and equity. Research objectives include improving primary care in terms of quality, safety, access, affordability, and equity; harnessing data and technology to conduct research on characteristics of primary care that may influence patient outcomes; management of clinical areas unique to primary care; focusing on clinical areas unique to primary care; addressing time-related issues and concerns in healthcare delivery; integrating behavioral health into primary care; using digital healthcare and technology to improve the delivery of high-quality primary care; implementation science to translate evidence more rapidly into primary care practice.

Eligibility Requirements
Eligible applicants include higher education institutions, nonprofits with 501(c)(3) IRS status (other than institutions of higher education), local governments, state governments, county governments, city or township governments, special district governments, Indian/Native American tribal governments (federally recognized), Indian/Native American tribal governments (other than federally recognized), federal governments, U.S. territory or possession entities. For-profit organizations are not eligible to lead applications under this research mechanism. Foreign institutions may participate in projects as members of consortia or as subcontractors only.

Period of Performance
The project period may not exceed 5 years.

Grant Value
The total costs for a project awarded under this NOFO will not exceed $500,000 in any given year and $2.5 million for the entire project period.
