The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $15,000,000 for Year 1, subject to the availability of funds.This NOFO aims to support health systems strengthening efforts and the delivery of HIV/TB prevention, care, and treatment services to meet the Joint United Nations Programme on HIV and AIDS 95-95-95 targets from the current 89-98-96 before 2030. Through technical assistance (TA) you are expected to:Use continuous quality improvement (CQI) approaches informed by program data to address poor antiretroviral therapy (ART) coverage.Information systems.Supply chain coordination and logistics management.Use surveys, evaluations, studies, and program data to identify and address program gaps in underserved, vulnerable, and key populations.Closing the gap in the first 95 among men who have sex with men, female sex workers, people who inject drugs, and other key populations in Zambia is the priority.Address the following gaps in the national HIV/AIDS response:Human resources for health (HRH).Laboratory capacity.Data management.Optimize HIV/TB service delivery quality management systems at designated sub-national levels.Develop and establish effective HIV/TB management and leadership capacity at the facility, district, and provincial levels to optimize program outcomes.

Background
The U.S. Centers for Disease Control and Prevention (CDC) is issuing this Notice of Funding Opportunity (NOFO) to provide targeted technical assistance to the HIV/TB prevention-care-treatment continuum in Zambia under the President’s Emergency Plan for AIDS Relief (PEPFAR). The goal is to support health systems strengthening efforts and the delivery of HIV/TB prevention, care, and treatment services to meet the Joint United Nations Programme on HIV and AIDS 95-95-95 targets by 2030. This program aims to address significant gaps in service delivery and improve health outcomes among vulnerable populations.

Grant Details
The grant will focus on several key tasks:

1) Use continuous quality improvement (CQI) approaches informed by program data to enhance antiretroviral therapy (ART) coverage;

2) Identify and address program gaps in underserved populations through surveys, evaluations, and studies;

3) Close the gap in ART coverage among key populations such as men who have sex with men, female sex workers, and people who inject drugs;

4) Address national HIV/AIDS response gaps including human resources for health, laboratory capacity, data management, information systems, and supply chain logistics;

5) Optimize service delivery quality management systems at sub-national levels;

6) Develop effective management and leadership capacity at various administrative levels to improve program outcomes.

Eligibility Requirements
This NOFO is open to a wide range of applicants including state governments, county governments, city or township governments, special district governments, independent school districts, public institutions of higher education, Native American tribal governments and organizations, nonprofits (both with and without 501(c)(3) status), private institutions of higher education, for-profit organizations (other than small businesses), small businesses, and foreign entities. There are no restrictions based on race, color, national origin, disability, age, sex (including gender identity), or other protected statuses.

Period of Performance
The expected period of performance is five years with projects awarded for five 12-month budget periods. The anticipated start date for the first year is September 30, 2025.

Grant Value
The total funding amount available for Year 1 is approximately $15 million. The expected number of awards ranges from one to three based on available funds and program priorities.

Place of Performance
The project must be conducted in Zambia.
