The purpose of this notice of funding opportunity (NOFO) is to build on the National Center for Chronic Disease Prevention and Health Promotion's (NCCDPHP) efforts to strengthen the capacity and capabilities of the states' chronic disease prevention and health promotion public health workforce. This program is expected to result in adoption of evidence-based chronic disease prevention and health promotion approaches and effective use of public health tools and resources. This 5-year NOFO supports strengthening the leadership and expertise in the chronic disease prevention system domains in order to transform the practice of chronic disease prevention and health promotion in state health departments and accelerate improvements in population health. Focusing on the priorities and goals of the NCCDPHP, this program will support the development of effective state chronic disease programs through implementation of practice-based learning and leadership development strategies.
Background
The purpose of this notice of funding opportunity (NOFO) is to build on the National Center for Chronic Disease Prevention and Health Promotion’s (NCCDPHP) efforts to strengthen the capacity and capabilities of the states’ chronic disease prevention and health promotion public health workforce. This program is expected to result in adoption of evidence-based chronic disease prevention and health promotion approaches and effective use of public health tools and resources.

Focusing on the priorities and goals of NCCDPHP, this program will support the development of effective state chronic disease programs through implementation of practice-based learning and leadership development strategies.

Grant Details
The public health workforce is facing many challenges, including retirement of the current workforce, evolving public health practice, changing public health needs that do not match current skills and capacity, lack of workforce strategic planning, turnover and burden of managing chronic disease programs during the COVID-19 pandemic.

In CDC's Modernizing the Workforce for the Public's Health: Shifting the Balance, contributors representing academic, governmental, and non-governmental public health workers concluded that the workforce would benefit from training, mentoring and cross-training, as well as leadership development. The NOFO will focus on strengthening the capacity and capabilities of the public health workforce, with particularly focus on health department staff across the United States, territories, and freely associated states.

It will support the development of effective state chronic disease programs through peer-to-peer meetings, practice-based learning and leadership development activities for chronic disease leadership and staff, and outreach and networking with local health departments.

Eligibility Requirements
Eligible applicants include state governments or their bona fide agents, local governments or their bona fide agents, territorial governments or their bona fide agents in various territories including American Samoa and Guam, state controlled institutions of higher education, American Indian or Alaska Native tribal governments (federally recognized or state-recognized), American Indian or Alaska native tribally designated organizations, ministries of health.

Period of Performance
The total period of performance funding is $15,000,000 with an average one year award amount of $3,000,000. The period of performance length is 5 years.

Place of Performance
The NOFO will improve and expand the leadership abilities and expertise of the state health department chronic disease prevention workforce resulting in chronic disease population health improvements in state health department catchment areas.
