The FY 2027 Health Center Program Service Area Competition (SAC) funding improves the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. For each SAC Notice of Funding Opportunity (NOFO), service areas available for competition are announced in the Service Area Announcement Table (SAAT). Throughout the year, we may announce additional service areas in a Service Area Competition-Additional Area (SAC-AA) if an awarded health center cannot continue to provide services in the funded service area.

Background
The Fiscal Year (FY) 2027 Health Center Program Service Area Competition (SAC) funding aims to improve the health of medically underserved communities and populations by providing grants to support the delivery of comprehensive, high-quality preventive and primary health care services in the United States and its territories. The Health Resources and Services Administration (HRSA) is responsible for this funding, which is intended to ensure access to affordable care while addressing pressing health challenges through chronic disease prevention and data-supported programs.

Grant Details
The FY 2027 SAC will provide grants to support health centers in delivering comprehensive primary and preventive health care services. Applicants must propose to serve at least one medically underserved area or population as defined in the Service Area Announcement Table (SAAT). The required services include prevention and management of chronic conditions, cancer screening, mental health services, and patient support services. Applicants must operate at least one full-time service delivery site in the proposed service area, open for patient care at least 40 hours per week. Funding awards will depend on appropriations and may vary based on the number of patients served. Organizations must demonstrate compliance with all Health Center Program requirements as outlined in the Health Center Program Compliance Manual.

Eligible applicants include state governments, county governments, city or township governments, special district governments, Native American tribal governments (Federally recognized), public housing authorities/Indian housing authorities, nonprofits with 501(c)(3) status (other than institutions of higher education), and other domestic organizations. Individuals are not eligible to apply. Applicants must propose to serve at least 75% of the SAAT Patient Target during the assessment period.

Period of Performance
