PurposeThe intent of this funding opportunity is to award entities to focus on specific targeted partnerships and innovative strategies to reduce falls and/or falls risk among the majority being older adults (60 years and older) and adults with disabilities, with a focus on those with greatest social and/or greatest economic needs (i.e., target population for this Funding) through the implementation of evidence-based falls prevention programs with an innovative person-centered approach.This funding opportunity has two goals:Goal 1: Develop an innovative fall prevention method that includes targeted partnerships and outreach to recruit the target population into small group in-person evidence-based falls prevention programs.Goal 2: Design an evaluation of your intervention to assess the impact of the effort on reducing falls and/or the risk of falls, and develop and disseminate 508-compliant program results, resources, lessons learned, and process documents to inform practice, service delivery, program development and/or policymaking for other organizations to replicate this approach, to include a comprehensive list of partnerships developed.The Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award 2 cooperative agreements with a three-year project period, subject to availability of funds.

Background
The Administration for Community Living (ACL) aims to maximize the independence, well-being, and health of older adults and people with disabilities. Through this funding opportunity, the Administration on Aging (AoA) plans to award two Cooperative Agreements to domestic public or private non-profit entities. The intent is to implement evidence-based falls prevention programs targeting older adults (60 years and older) and adults with disabilities, particularly those facing significant social and economic challenges.

Grant Details
The funding opportunity has two main goals:

1) Develop an innovative fall prevention method that includes targeted partnerships and outreach to recruit the target population into small group in-person evidence-based falls prevention programs. This includes delivering two or more workshops, with at least one program offered remotely. The approach must be person-centered, trauma-informed, and culturally appropriate.

2) Design an evaluation of the intervention to assess its impact on reducing falls or fall risks. This involves creating 508-compliant resources to disseminate results and lessons learned for replication by other organizations. Applicants must engage approximately 25% of target participants by Year 1, 50% by Year 2, and 100% by Year 3.

Eligibility Requirements
Eligible applicants include domestic public or private non-profit entities such as state governments, Native American tribal organizations (not federally recognized), public housing authorities, nonprofits with a 501(C)(3) status (excluding institutions of higher education), city or township governments, public and state-controlled institutes of higher education, independent school districts, private institutions of higher education, county governments, faith-based organizations, and community organizations.

Period of Performance
The project period is set for three years with an anticipated start date of July 1, 2025.

Grant Value
The total estimated funding available is $2,500,000 with each award having a ceiling of $1,250,000 for the project period.

Place of Performance
The specific geographic locations for performance should focus on areas with significant populations of older adults and adults with disabilities facing social or economic challenges.
