The NIH INvestigation of Co-occurring conditions across the Lifespan to Understand Down syndromE (INCLUDE) Project seeks to improve health and quality-of-life for individuals with Down syndrome. This Notice of Funding Opportunity (NOFO) is soliciting applications for a Down Syndrome Clinical Cohort Coordinating Center (DS-4C) that will be responsible for the overall coordination and outreach for the new INCLUDE Project Down Syndrome Cohort Development Program (DS-CDP), support development of new cohorts of people with Down syndrome, and curate the metadata from the new cohorts. A companion NOFO (RFA-OD-23-052) will support the Cohort Research Sites. A related NOFO (RFA-OD-23-054) will support a Federated Biobanking Resource for the INCLUDE Project. This NOFO requires a Plan for Enhancing Diverse Perspectives (PEDP), which will be assessed as part of the scientific and technical peer review evaluation. Applications that fail to include a PEDP will be considered incomplete and will be withdrawn. Background
The National Institutes of Health (NIH) is seeking to improve health and quality-of-life for individuals with Down syndrome through the INvestigation of Co-occurring conditions across the Lifespan to Understand Down syndromE (INCLUDE) Project. The Down Syndrome Clinical Cohort Coordinating Center (DS-4C) is responsible for overall coordination and outreach for the new INCLUDE Project Down Syndrome Cohort Development Program (DS-CDP), support development of new cohorts of people with Down syndrome, and curate the metadata from the new cohorts.

Grant Details
The DS-4C will provide overall management and oversight for the DS cohort development program, including coordination and communication across all subsections and cores of the program. It will facilitate data sharing within the research community through the INCLUDE Data Coordinating Center (DCC).

The DS-4C will work in concert with the Down Syndrome Cohort Research Sites (DS-CRS) to recruit participants with DS across the lifespan, collect biospecimens, and assemble phenotypic data, images, and omics for deep phenotyping. The DS-4C will also coordinate and implement EHR data acquisition from each Research Site to collect baseline and longitudinal data on enrolled participants.

Eligibility Requirements
Eligible applicants include higher education institutions, nonprofits, for-profit organizations, state and local governments, federal agencies, U.S. territories or possessions, and foreign components of U.S. organizations. Individuals from diverse backgrounds are encouraged to apply.

Period of Performance
The project period is a maximum of five years. The earliest start date is January 26, 2024, with an expiration date of January 27, 2024.

Grant Value
The estimated total funding commitment is $2 million in year 1 and $4 million in subsequent years to fund one award. Application budgets are not limited but need to reflect the actual needs of the proposed project.
