The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $7,000,000 for Year 1, subject to the availability of funds. This NOFO is intended to support implementation of a comprehensive public health response to new HIV infections for sustained epidemic control in Lesotho. The program will engage in comprehensive and evidence-based community HIV prevention, testing and treatment initiation for the general population and other at-risk groups such as youth, men, migrant, and factory workers, as well as building capacity of local organizations, civil society organizations and community systems in implementing data-driven programming. The project will use mapping of hot spots as informed by facility and community-based surveillance, recency, and other HIV programmatic and epidemiological data; combined with same-day onsite anti-retroviral treatment (ART) initiation for identified persons living with HIV (PLHIV), and timely pre-exposure prophylaxis (PrEP) initiation and/or linkages to voluntary medical male circumcision (VMMC) for eligible HIV-negative clients; as well as PEP provision for survivors of gender based violence. The project will also offer comprehensive community HIV prevention services targeting adolescent girls and young women (AGYW) and their sexual partners with risk communication, HIV testing, sexual and reproductive health services, and linkages to socio-economic strengthening programs under the DREAMS initiative.

Background
The UNAIDS Global AIDS Strategy 2021-2026 sets out evidence-based priority actions and targets for every country to end AIDS as a public health threat by 2030. The recent Lesotho Population-based HIV Impact Assessment (LePHIA, 2020) revealed that Lesotho has made significant strides towards controlling the HIV epidemic in the last few years. Although these results place Lesotho on track to achieve the UNAIDS 95-95-95 targets by 2025, gaps remain in the clinical cascade across sex and age bands, underscoring the need to address barriers that affect access, uptake, and retention.

Grant Details
This NOFO is intended to support the government of Lesotho to reach and sustain HIV epidemic control by expanding access to hard-to-find PLHIV in communities and improving access to high impact community-based HIV prevention, treatment, and support services.
The project will utilize real-time program and survey data to map clusters of new HIV infections by geography and population, and deliver high impact evidence-based HIV prevention, testing and treatment services to ensure Lesotho ends HIV/AIDS by 2030.
