Government Description
MEDICAL CASE MANAGEMENT SERVICES PROVIDED TO SUPPORT INDIVIDUAL MEDICAL READINESS (IMR) FOR SOLDIERS. THE CONTRACTOR DELIVERS MEDICAL NON-CLINICAL CASE MANAGEMENT AND ADMINISTRATIVE CARE COORDINATOR STAFFING, MANAGING A SOLDIER’S CARE FROM IDENTIFICATION OF A MEDICAL READINESS CONDITION THROUGH TO MEDICAL DEPLOYMENT READINESS OR ADMINISTRATIVE SEPARATION. SERVICES ARE PERFORMED IN ACCORDANCE WITH DEFENSE HEALTH AGENCY (DHA) PROCEDURAL INSTRUCTION 6025.20, SUPPORTING THE ILLINOIS ARMY NATIONAL GUARD (ILARNG) SURGEON’S OFFICE ACROSS FIVE BRIGADES. ALL NECESSARY PERSONNEL, EQUIPMENT, TOOLS, MATERIALS, SUPERVISION, AND QUALITY CONTROL ARE SUPPLIED TO FULFILL THE ANNUAL MISSION REQUIREMENTS.
