Demo, Remove, Replace Flooring 8N Mental Health   2

Background
The Oklahoma City VA Health Care System is issuing this contract to replace the flooring in the 8 North Mental Health Unit. The goal of this contract is to ensure a safe, clean, and functional environment for patients and staff by replacing significantly worn flooring that has deep scratches and areas of deterioration.

Work Details
The contractor shall furnish all labor, supervision, materials, equipment, and tools necessary to remove existing flooring and install new flooring in the 8 North Mental Health Unit. Specific tasks include:

- Removing approximately 3,200 sq. ft. of existing sheet vinyl flooring and flash cove;
- Preparing the area for new sheet vinyl flooring according to manufacturer installation instructions;
- Welding all seams for the new sheet vinyl;
- Installing approximately 3,200 sq. ft. of new VA-approved sheet vinyl flooring and 6-inch flash cove;
- Ensuring a distinct color change on the ramp leading to the Patient waiting area;
- Supplying all materials needed for the project;
- Disposing of any generated trash or debris; and
- Cleaning up the area after project completion.

Period of Performance
30 days from the start date of the contract.

Oklahoma City VA Health Care System.

Bidder Requirements
Bidders must be certified as Service-Disabled Veteran-Owned Small Business (SDVOSB) and provide past performance information on two relevant projects completed within the last five years, each valued at $25,000 or more. Contractors must have an Experience Modification Rate (EMR) of 1.0 or less, or provide an explanation if it exceeds this threshold. Compliance with OSHA or EPA regulations is required, with no more than three serious violations in the past three years.
