Design-Build Renovate 3rd Floor for Oncology & Telehealth   3

Provide all supervision, labor, materials, tools, equipment, and design-build A-E services necessary for design and construction of an Oncology & Tele-Health Suite Renovation project described by this RFP.

Project Scope Summary

The Design Build team shall provide all, labor, materials, tools and equipment, and design-build services necessary for design and construction of a project described here along with other specified tasks as further defined in a request for proposal (RFP) for renovation of Project No. 562-18-407, Renovate 3rd Floor for Oncology & Telehealth. The project shall be designed and completed in accordance with current VA Criteria, EPA requirements, VA Specifications and developed specifically to meet local requirements for the Erie VAMC in Erie PA. The resultant contract of this procurement is to provide required design completion of the referenced project and perform construction of completed design work.

2/16/2021 - Amendment 0001 - Corrects Erie VAMC zip code and notifies potential offerors of 2/23/2021 site visit.

2/22/2021 - Amendment 0002 - Adds corrected Exhibit E, adds base layout drawing,and reminds offerors of pre-sol site visit.

3/15/2021 - Amendment 0003 - Extends the due date and provides clarifications to the solicitation.

3/23/2021 - Amendment 0004 - Extends the due date to 3/31/2021 and clarifies solicitation exhibits

3/24/2021 - Amendment 0005 - Corrects amendment 0004 comment on Exhibit E
