10 APR 25 - Solicitation Amendment 25RE001-0001

This amendment provides updates to several items of the RFP package in response to the questions received from industry on 27 March. Please note that the proposal due dates and milestones have been updated in Section L. See Section J for a full list of attachments, and identification of items that were updated.

8 APR 25 UPDATE: There has been significant interest in this requirement. The Government is working to respond to the number of questions received and provide updates to the solicitation documents as needed. This process is taking longer than expected. As such, the proposal submission date has been extended to 18 April 2025. The formal SF33 is attached to this update. The next update will include Amendment 01 and all revised documents. We appreciate your patience and understanding.

NOTE: ATTACHMENT 02- FORMAL SF33 WILL BE POSTED TO THIS SOLICITATION VIA AMENDMENT AS SOON AS AVAILABLE. Proposal submission date will not be adjusted based on posting of SF33.

The U.S. Special Operations Command (USSOCOM) Warrior Care Program (WCP) provides non-clinical case management and advocacy services for Special Operations Forces (SOF) wounded, ill, or injured (WII) service members. The focus areas are in Recovery Care Coordination, Military Adaptive Sports, Benevolence, and Career Transition. Services are extended to families, veterans, caregivers, and significant others of SOF WII. The Warrior Care Global Support (WCGS) effort is a non-personal service requirement in support of USSOCOM WCP. The program will provide support services to the USSOCOM Enterprise.

Background
The U.S. Special Operations Command (USSOCOM) Warrior Care Program (WCP) is dedicated to providing non-clinical case management and advocacy services for Special Operations Forces (SOF) service members who are wounded, ill, or injured. The program focuses on Recovery Care Coordination, Military Adaptive Sports, Benevolence, and Career Transition, extending its services to families, veterans, caregivers, and significant others of SOF WII.

The Warrior Care Global Support (WCGS) initiative is a non-personal service requirement aimed at supporting the USSOCOM WCP by delivering essential support services across various commands both within the Continental United States (CONUS) and Outside the Continental United States (OCONUS).

Work Details
The contractor shall provide full-time personnel to support the following focus areas: Recovery Care Operations, Military Adaptive Sports, Benevolence, and Career Transition. Responsibilities include:

1. Executing a transition plan ensuring qualified personnel are in place from day one of performance without interrupting mission operations.
2. Providing coordination on non-clinical healthcare and social services to assist clients in navigating available resources effectively.
3. Ensuring all personnel meet required skills and qualifications as specified in the Statement of Work (SOW).
4. Facilitating comprehensive screening, assessment, planning, implementation, coordination, monitoring, and evaluation of services provided to enrolled participants.

Period of Performance
The contract will be performed over a base period of 12 months with four additional 12-month option periods.

Place of Performance
The contract will be executed at various locations both within the Continental United States (CONUS) and Outside the Continental United States (OCONUS), specifically supporting Headquarters USSOCOM and its Component Commands.
