WAPA DSW Medical Support Services

Background
The Western Area Power Administration- Desert Southwest Region (WAPA-DSW) has a requirement for medical support services in various locations in Arizona and Henderson, Nevada. The contract will involve providing periodic occupational health examinations, exit medical examinations, and fit for duty assessments for employees of WAPA in and around Arizona and Nevada. The contract will be awarded with a base year and four option years.

Work Details
The Contractor is required to provide all necessary facilities, staff, materials, and equipment to conduct the specified medical examinations. The scope of work is outlined in each specific exam packet located in Attachment B (WAPA Order 340.1). The performance under this contract shall primarily occur at the DSW Regional Office located at 615 S. 43rd Avenue, Phoenix, Arizona.
Additionally, the Contractor must be able to provide service to employees located in various locations including Yuma, Flagstaff, Phoenix, Casa Grande, Gilbert, Lake Havasu, Mesa in Arizona, and Las Vegas and Henderson in Nevada.

Period of Performance
The contract shall be awarded with a base year and with four option years.

Place of Performance
Performance under this contract shall primarily occur at the DSW Regional Office located at 615 S. 43rd Avenue, Phoenix, Arizona.
