Background
The Department of Veterans Affairs (VA) is issuing this solicitation to provide Durable Medical Equipment (DME) services for patients in the Veterans Integrated Service Network (VISN) 10, which encompasses Indiana and Michigan. The goal of this contract is to ensure that prescribed, government-owned DME is delivered to patients' residences, thereby supporting the VA's mission to provide high-quality healthcare to veterans.

Work Details
The contractor shall furnish all necessary labor, materials, and equipment to assemble, clean, refurbish, and store DME equipment. Services include delivery, set-up, repair, in-house instruction and patient education, and pick-up of government-owned items for beneficiaries. The contractor must be available for routine contact Monday through Friday from 8:00 AM to 4:30 PM and provide emergency services 24/7.

The contractor will also develop a Performance Monitoring Plan and Quality Assurance Surveillance Plan to ensure compliance with service standards. Specific tasks include:
1. Delivery of DME within two working days of order placement.
2. Providing instructions on the use and care of equipment.
3. Conducting re-education visits at no cost if concerns arise within 90 days post-delivery.
4. Maintaining a weekly delivery schedule from the warehouse to each VA facility or Community-Based Outpatient Clinic (CBOC).

Period of Performance
The contract will have a base period of one year with four optional years that may be exercised at the discretion of the VA.

Place of Performance
The services will be performed in various counties across Indiana and Michigan as specified under the geographic service areas associated with different VA Medical Centers.
