Background
The Defense Health Agency, Professional Services – Contracting Division, is soliciting proposals for the Vision Center of Excellence, Vision Care Coordination requirement to provide a nationwide network for the coordination of vision health care in the Department of Defense Military Health System.

The Military Health System (MHS) provides comprehensive vision care for Department of Defense (DoD) beneficiaries, which may involve multiple eye specialists not present at every Military Treatment Facility (MTF). This contract aims to ensure timely and appropriate vision care through a system of coordination among specialists.

Work Details
The contractor shall provide program management support services including:

1. Arranging eye care for patients (referrals, movement of patients, follow-ups, monitoring performance metrics).
2. Establishing a nationwide network consisting of four regional vision care coordinators located at Ocular Trauma Centers or MTFs and one national coordinator at the Vision Center of Excellence.
3. Facilitating patient movement between DoD and Veterans Affairs to maximize available vision care.

Specific tasks include:
- Task 5.1 – Vision Care Coordination
- Task 5.1.2.1 Patient Outreach
- Task 5.1.2.7 Referrals and Consultations
- Task 5.1.3 – National Vision Care Coordination
- Task 5.1.4 – Collection of Health Care Data.

Period of Performance
One twelve-month base year plus four twelve-month option periods: Base Year: 09/24/2025 – 09/23/2026; Option Year 1: 09/24/2026 – 09/23/2027; Option Year 2: 09/24/2027 – 09/23/2028; Option Year 3: 09/24/2028 – 09/23/2029; Option Year 4: 09/24/2029 – 09/23/2030.

Place of Performance
The work will be performed nationwide with specific regional coordinators located at San Antonio Military Medical Center, San Diego Naval Medical Center, Madigan Army Medical Center, and Walter Reed National Military Medical Center.
