VA Roseburg Healthcare System Fixed Dental Prosthetics   3

Background
The Department of Veterans Affairs (VA) is issuing solicitation number 36C26026Q0529 as a Request for Quotation for the provision of fixed dental prosthetics to the VA Roseburg Healthcare System (VARHCS). This acquisition is set aside for small business concerns, with a focus on providing high-quality dental laboratory support to meet the needs of veterans.

The goal of this contract is to ensure that the VARHCS Dental Service receives timely and efficient fabrication and delivery of dental prosthetics, thereby enhancing patient care.

Work Details
The Contractor shall fabricate high-quality dental prosthetics for the VARHCS Dental Service and provide transportation and delivery services directly to and from the Roseburg and Eugene clinics. The Contractor will be responsible for all aspects of fabrication, including:
- Transportation, shipping, delivery, labor, supplies, supervision, quality assurance, and materials required to fabricate dental prosthetics in accordance with dental laboratory prescription specifications.
- All materials must comply with FDA standards and ANSI regulations.
- The Contractor must provide a full-service dental laboratory capable of integrating treatment needs for patients requiring fixed and implant-supported prostheses.

Specific items include:
1. Full-Cast Crown Inlay/Onlay Yellow Gold
2. Full Gold Crown
3. Porcelain fused to metal crowns/bridges
4. Zirconia restorations
5. Diagnostic wax-ups
6. Various implant-related services including surgical guides and custom abutments.
Each product must be delivered in a sealed plastic envelope with appropriate labeling including patient information.

Period of Performance
Base Year: 05/01/2026 - 04/30/2027; Option Year One: 05/01/2027 - 04/30/2028; Option Year Two: 05/01/2028 - 04/30/2029; Option Year Three: 05/01/2029 - 04/30/2030; Option Year Four: 05/01/2030 - 04/30/2031.

Place of Performance
Services will be performed at the Contractor's dental lab site or its specialized subcontractors, with deliveries made to the VA Roseburg Healthcare System located at 913 NW Garden Valley Blvd, Roseburg, OR 97471, and Eugene Healthcare Center at 3355 Chad Dr, Eugene, OR 97408.
