This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The solicitation number is 36C25722Q0415. This acquisition is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04, effective January 30, 2022. The associated North American Industrial Classification System Code (NAICS) for this procurement is 485991 with a small business standard of $16.5M. This acquisition is solicited as a 100% set-aside for Service-Disabled Veteran Owen Small Business. PLEASE READ ALLATTACHMENTS
Background
This is a Combined Synopsis/Solicitation (RFQ 36C25722Q0415) issued by the Department of Veterans Affairs, VISN 17 Network Contracting Activity, for commercial wheelchair transportation services for veterans.
The acquisition is a 100% set-aside for Service-Disabled Veteran-Owned Small Business (SDVOSBC).

Work Details
Contractor shall provide all labor, material, supplies, equipment, and supervision to perform Wheelchair Transportation Services to/from Dallas VA Medical Center (4500 S. Lancaster Rd, Dallas, TX 75216), community care appointments, community hospitals and patient residences.
Services include: 24/7, 365-day operations; contingency planning for service interruptions; receipt and dispatch of daily manifests; communication via two-way radio or cellular; secure lift operation with emergency backup; climate-controlled wheelchair vans with stretcher tie-down straps and secure devices; compliance with Federal, State and local safety, licensing and registration requirements; preventive maintenance per manufacturer schedule; CDC infection control guidance; secure seat belts and stretcher locks; environmental management system conformance; and post-award orientation teleconference participation.
The contract is IDIQ fixed-price with a $3,000 guaranteed minimum and $6,000,000 maximum aggregate value.

Period of Performance
Base Period: 04-01-2022 to 03-31-2023; Option 1: 04-01-2023 to 03-31-2024; Option 2: 04-01-2024 to 03-31-2025; Option 3: 04-01-2025 to 03-31-2026; Option 4: 04-01-2026 to 03-31-2027

Place of Performance
Dallas VA Medical Center, 4500 S. Lancaster Road, Dallas, Texas 75216; including all Community Care appointments, community hospitals and VA facilities within and outside the service area, and patient residences as directed by the COR.

Bidder Requirements
100% SDVOSBC set-aside; offerors must be verified in VA VIP database at time of submission and award; comply with SBA limitations on subcontracting (13 CFR 125.6).
Submit in PDF: list of transportation vehicles (year, make, model, VIN, capabilities), contingency plan, dispatch procedures; provide minimum three past performance references from last three years.
Vehicles must meet Federal and Texas Health & Safety Code and Texas Administrative Code Chapter 157 requirements, with insurance, licensing, registration, operational air conditioning/heating, two-way radio/cellular communication, stretcher tie-down straps, lift with emergency backup; drivers must hold a Texas Class D operator license; ALS vehicles require an on-board Paramedic with ALS equipment; BLS vehicles require an on-board EMT; all on-site personnel must complete yearly EPA environmental training.
