Background
The contract is for Wheelchair Transportation Service at Cleveland VAMC and CBOC. The contractor shall provide all required labor, material, supplies, equipment, and supervision of contractor personnel necessary to perform Wheelchair Transportation Service for Cleveland Veterans Affairs Medical Center (VAMC) and surrounding Clinics identified in PWS. This solicitation is anticipated to result in the award of a FP with EPA contract for a base plus two option years.

Work Details
The contractor shall provide wheelchair transportation service for beneficiaries including physically or mentally handicapped individuals in wheelchairs and aged and infirm persons, their attendants, and personal belongings between points in Ohio. The contractor shall adhere to Ohio Administrative Code (OAC), State Board of Emergency Medical, Fire, and Transportation Services (EMFTS) and all applicable Federal, State and Local laws, rules, regulations, and procedures as required to perform services under this contract.
The contractor shall furnish the vehicles necessary to provide full and complete coverage for pick-up and delivery of all Veteran patients requiring transport the of this contract for the locations specified herein.

Period of Performance
The contract period includes a base period of performance beginning on 07-01-2023 and ending on 06-30-2024. Additionally, there are two option years: Option 1 period of performance begins on 07-01-2024 and ends on 06-30-2025; Option 2 period of performance begins on 07-01-2025 and ends on 06-30-2026.

Place of Performance
The geographic locations where the contract will be performed include Cleveland VA Medical Center (Wade Park Campus), Akron Community Based Outpatient Clinic, Canton Community Based Outpatient Clinic, Ravenna Community Based Outpatient Clinic, New Philadelphia Community Based Outpatient Clinic, Mansfield Community Based Outpatient Clinic, Lake County VA Facility, Sandusky Community Based Outpatient Clinic, Lorain Community Based Outpatient Clinic, Parma Community Based Outpatient Clinic, East Liverpool Community Based Outpatient Clinic, Warren Community Based Outpatient Clinic, Youngstown Community Based Outpatient Clinic.
