Instructions to interested parties. Due to an RFQ protest, investigation as to the need of an Emergency Medical and Fire Transportation Service (EMFTS) license is requirement. Also, additional market research was required to determine competition. It has been determined that an EMFTS license is a minimum need to conduct wheelchair transport service in Ohio. A sources sought notice was published to evaluate the market to determine the set-aside. It has been determined by market research that to ensure adequate competition a total small business set-aside is required. To remain in competition for this award the following information shall be completed and submitted. Any quote submissions before the publish date of Amendment 0003 will not be considered for award. Items to be submitted are listed in RFQ under section E.11 (pp. 71-72)

Background
The Department of Veterans Affairs (VA) is seeking a contractor to provide wheelchair transportation services for the Cincinnati VA Medical Center (VAMC) and its affiliated locations. The contract will be performed in accordance with Subpart 13.5 and is expected to result in the award of an Indefinite-Delivery type contract as defined in Subpart 16.5. The performance of services under this contract will be authorized through the award of funded task order(s) by the government contracting office. The anticipated performance periods are as follows: Base Year from 1 August 2024 to 31 October 2024; Option Year One from 1 November 2024 to 31 October 2025; Option Year Two from 1 November 2025 to 31 October 2026.

Work Details
The contractor is required to provide all necessary labor, material, supplies, equipment, and supervision of contractor personnel to perform wheelchair transportation services for the Cincinnati VAMC and its affiliated locations. Trips are not restricted to specific counties and are anticipated to be needed between the hours of 7 am and 7 pm Monday through Friday, with limited services possibly needed at any time, including weekends and holidays. The contract will be for 24-hour service, seven days per week. The contractor shall develop and maintain an effective quality control program to ensure compliance with all contract requirements.
The contractor's quality control program shall include an inspection plan, methods for identifying and preventing deficiencies in service quality, on-site records of vehicle maintenance and repairs, records of employee training and certifications, methods for identifying and preventing radio communication breakdowns, a log to account for all service requests, records of complaints or problems with procedures taken for corrections, among other requirements.

Period of Performance
The performance period includes the Base Period from 1 August 2024 to 31 October 2024, Option Year One from 1 November 2024 to 31 October 2025, and Option Year Two from 1 November 2025 to 31 October 2026.

Place of Performance
The contract work will be performed at the Cincinnati VA Medical Center (VAMC) in Ohio and its affiliated locations: Nursing Care Unit and Domiciliary at Ft. Thomas, KY; Community Based Outpatient Clinics (Dearborn County, IN; Campbell County and Boone County, KY; Brown County, Butler County, and Clermont County, OH).
