Caregiver Program Support Services for VHA   4

Indefinite-Delivery Requirements Contract for the provision of the VA National Caregiver Support Program training, education, and support services for caregivers of veterans. Contract Period: 5 Year Ordering Period Task orders issued under this contract will identify their own specific period of performance.

OFFEROR MUST ENSURE THAT ITS PROPOSAL IS SUBMITTED TIMELEY TO ENSURE DELIVERY AND RECEIPT BY THE DUE DATE AND TIME IDENTIFIED IN BOX 20 ON THE SF1449 (Page 1 of the solicitation). LATE PROPOSALS WILL NOT BE ACCEPTED.

Offeror must:

(a) Complete Box 12, 17, and 30 of the SF 1449 (Page 1) of the solicitation. Provide the legal entity name, address, telephone number and SAM Unique identifier in Box 17.

(b) Complete Section B.1., contractor POC information.

(c) Complete Price/Cost Schedule: Offeror must enter a proposed price for each CLIN in the attached Excel Spreadsheet document titled Line Items for Caregiver Contract. Offeror may separately include a narrative explanation of the criteria, assumptions and the like that was taken into consideration to arrive at the proposed prices.

(d) Provide information that clearly describes your company's technical capability to perform the requirements of the PWS as demonstrated through the following:

Management Plan - The Offeror's management plan must, at a minimum, clearly specify the technical approach, method, or plan that focuses on accomplishing the requirements outlined in the PWS.

Staffing Plan - The Offeror's staffing plan must, at a minimum, clearly specify the technical approach, method, or plan to ensure the proposed staffing levels will meet the requirements of the contract and enable successful implementation of training services.

Training Platform - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, or plan for the implementation of a new training website and how the website will meet the training needs of caregivers and adhere to VA Information Security requirements.

Quality Control Plan (QCP) - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, or plan to establish an internal QCP taking into account the criteria outlined in the attached Quality Assurance Surveillance Plan (QASP).

Satisfaction Surveys - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, or plan to distribute the Satisfaction Survey outlined in task 4 of the PWS.

Referral - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, or plan to establish and provide a referral website to meet the requirements outlined in Task 3 of the PWS.

National Caregiver Training (NCT) - The Offeror's NCT must, at a minimum, clearly specify the technical approach, method, or plan to execute the implementation, provisions, and requirements outlined in Task 6 of the PWS.

Language Translation - The Offeror's Proposal must, at a minimum, clearly specify the technical approach, method, or plan for the provision of Language translation services in accordance with Task 9,10, and 12 of the PWS.

Marketing Materials - The Offeror's marketing materials must, at a minimum, clearly specify the technical approach, method, plan, and capability to develop informational flyers & brochures for training modalities outlined in Task 11 of the PWS.

Spanish Self-Care Course - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, plan to provide Spanish Self-Care course delivery as outlined in Task 9 of the PWS.

Spanish Translation for Peer Mentoring calls - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, plan to provide Spanish Translation for Peer Mentoring calls as outlined in Task 10 of the PWS.

Backup Planning - The Offeror's proposal must, at a minimum, clearly specify the technical approach, method, plan for contingency planning to address any unexpected website outages, mail delivery failures, staff shortages, that could disrupt caregiver training services.

(e) Provide information that demonstrates your company's PAST PERFORMANCE

1. Previous Experience: Provide information for up to 3 contracts awarded to your company by VHA or other healthcare entities within the past 5 years that are/were similar in size and scope to this requirement. Information for each contract must include (a) name of the contracting entity and a brief description of contract scope & requirements, (b) total dollar value, and (c) contract period of performance.

2. Provide CPARS rating(s) or any other past performance rating(s) provided by the Department of Veterans Affairs, or other Healthcare Entities, or other Federal Agencies.

(f) Provide the certificate required in VAAR 852.219-77: VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction.

(g)The Offeror shall complete FAR Provision 52.212-3. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v)) of this provision.

(h) Offerors who intend to perform in a place or area of performance for which a wage determination has not been attached must request wage determinations for additional places or areas of performance in writing no later than 08-22-2022 by 15:00hrs Eastern.

(i) Provide documentation that best describes how the offeror will ensure protection when storing, generating, transmitting, or exchanging VA sensitive information. In accordance with (IAW) the VA Handbook VA Handbook 6500.6 Appendix C VA information security requirements: For service that involves the storage, generating, transmitting, or exchanging of VA sensitive information but does not require C&A or an MOU-ISA for system interconnection, the contractor/subcontractor must then specify the mechanism/documentation used to ensure the VA sensitive information is protected. This is to be submitted to the PM/COR.

(j) Offeror must complete the contractor fill in sections of the attached Business Associate Agreement.

(k) Offeror must complete the contractor fill in sections of the attached Contractor Immigration & Indemnity Certification document.

(l) Offeror must complete the contractor fill in section of the FAR 52.209-7(b) Information Regarding Responsibility Matters.
