This solicitation is for health and life insurance for employees and dependents at the Embassy of the United States of America located in Tegucigalpa, Honduras, and possibly select other locations. The anticipated performance period is for a base year, with four one-year option periods, if exercised by the Government. All responsible sources may submit a quotation, which will be considered.

The Government may award the contract based on the initial offer without discussion. The resultant contract will be a firm fixed price type contract.

The Government is planning a pre-quotation conference and the date for this conference will be in the solicitation.
All contractors must be registered in the SAM (System for Award Management) Database https://www.sam.gov prior to submitting an offer pursuant to FAR provision 52.204-7. Therefore, prospective offerors are encouraged to register prior to the submittal of quotations/proposals.

This opportunity is subject to 52.229-12 - Tax on Certain Foreign Procurements (FEB 2021).

Please see Amendment 0002 and Amendment 0003, which do the following:

Amendment 0002:
- Extends Offer's Due Date from Monday, December 19, 2022 to Friday, December 30, 2022 at 10:00 a.m. (Local Time).

Amendment 0003:

Replace Page 5 to correct in Section B.3.2 the Categories of the Retention Table from "Employee", "Employee's Spouse", and
"Employee's Dependent Child" to "Self", "Self Plus One", and "Family" respectively.
- Replace Pages 9 and 10 to delete in Section B.7.2 Table of Retention the Column "Accidental Death and Dismemberment"
- Replace Page 69 to add the Tables LIFE CLAIMS PAID and AVERAGE INSURED MEMBER COUNTS for the last 3 years.

Background
The U.S. Embassy in Tegucigalpa, Honduras is soliciting a firm-fixed-price contract.
It is to provide health and life insurance coverage for its employees and eligible dependents, and possibly select other locations, in support of Mission Honduras.

Work Details
Provide comprehensive health insurance services in accordance with Section C: C.1.1 Employee and Dependent Health Service Benefits including:
- Hospitalization: 100% coverage of inpatient care, bed & board (semi-private), operating room, recovery room, ICU, imaging, diagnostic testing, nursing care, physical therapy, drugs and medicines administered inpatient.
- Ambulance: licensed ground transport at minimum 80% coverage.
- Emergency services: 100% coverage for emergency room visits.
- Outpatient services: minimum 80% coverage for annual physicals, specialist consultations, minor surgeries, chemotherapy/radiation, immunizations, diagnostic tests, diagnostic imaging, rehabilitative services, psychiatric therapy.
- Obstetric and newborn care: 100% inpatient, 80% outpatient.
- Prescription drugs: 100% inpatient, 80% outpatient.
- Pediatric services: 100% inpatient, 80% outpatient.
- Preventive and wellness services and chronic disease management: 80% coverage.
- Hearing aids: one apparatus per ear every three years up to HNL 36,960 per individual at 80% coverage.
- Dental care: 80% coverage for exams, cleanings, fillings, extractions, crowns, dentures.
- Optical care: 80% coverage up to HNL 7,392 per individual every two years.
Include riders for Official Residence Expense staff, JTF-Bravo, Embassy Association/AAFES employees, and retirees, billed separately.
Provide all documentation to support economic price adjustments and comply with performance standards: all services per PWS paragraphs C.1.0–C.1.6, with no more than one customer complaint per month and corrective action per the Inspection of Services clause.

Period of Performance
One-year base period beginning March 12, 2023, with four one-year option periods.

Place of Performance
Tegucigalpa, Honduras, including U.S. Embassy facilities, Official Residence, Joint Task Force-Bravo locations, and Embassy Association operations.
