The VA North Texas Health Care System is seeking to provide Community Based Outpatient Clinic (CBOC) services primarily for Veterans residing in Grayson County, Texas, and surrounding areas. The goal of this contract is to ensure that Veterans have access to necessary outpatient services in a private hospital, office, or clinic environment. This initiative aligns with the VA's mission to enhance healthcare delivery and improve access to care for Veterans.

Work Details
The contractor will be responsible for providing outpatient clinic services including but not limited to: primary care, mental health services, coordination of care, and adherence to VHA performance and quality criteria. Services must comply with established standards regarding access, quality of care, Veteran satisfaction, and clinical guidelines.

The contractor must also ensure proper staffing levels as per VA standards, including a sufficient number of primary care providers and support staff. The contract will operate under a fixed-price indefinite-delivery indefinite-quantity (IDIQ) model with task orders issued for specific services. The contractor is required to maintain a monthly billable roster of patients and adhere to billing procedures outlined in the Performance Work Statement (PWS).
