Amendment 006 makes changes to the Solicitation, answers a question, and provides an updated Attachment J-11: Pricing Template.

Amendment 005 exends the date for submission of proposals from May 15, 2023 at 12:00 PM to May 30, 2023 at 12:00 PM.

Amendment 004 replaces Attachment J-11: Pricing Template in order to correct the description.

Amendment 003 makes changes to Section B and replaces Attachment J-11: Pricing Template in order to correct a Quantity discrepancy in the pricing tables and to clarify the basis as to how the Quantity figures were calculated.

Amendment 002 makes changes to the Section C, Statement of Work, requirements, and a change to Attachment J-5 is also included in this Amendment.

Amendment 001 provides answers to questions asked and provides a new Section H.15 Anti-Harassment. There are no other changes to the solicitation.

The objective of this contract is for the Contractor to provide Supportive Housing services (up to 180 days), within a 50-mile radius of 633 Indiana Avenue, NW, Washington, DC, to prepare offenders for independent living and improved capacity for employment and housing stability. A successful participant will demonstrate progress towards employment and the ability to contribute financially to the cost of stable housing wherever they may reside. This includes placement in an independent living program.

Background
The Court Services and Offender Supervision Agency (CSOSA) was established in August 1997 to supervise adults under its jurisdiction, enhance public safety, reduce recidivism, and promote accountability through evidence-based practices.

The objective of this contract is to provide Supportive Housing services for offenders within a 50-mile radius of 633 Indiana Avenue, NW, Washington, DC. These services aim to prepare offenders for independent living and improve their capacity for employment and housing stability.

Work Details
The Contractor shall provide Supportive Housing services for up to 180 days, including:
1. **Intake Process**: Transport offenders, conduct intake interviews within 24 hours of admission, and create individual case management files.
2. **Supportive Housing Programming Services**: Provide case management, workforce development skills, life skills training, recreational education, and referrals to vocational training.
3. **Discharge Planning**: Conduct discharge staffings and prepare comprehensive discharge plans summarizing the offender's progress and future housing plans.
4. **Ongoing Case Management**: Maintain weekly progress notes reflecting the offender's response to programming and document interactions with CSO (Community Supervision Officer).
5. **Compliance with Regulations**: Ensure adherence to HIPAA privacy rules and CSOSA security requirements while handling Personally Identifiable Information (PII).

Period of Performance
Base Period of 12 months following award with four additional option periods of 12 months each.

Place of Performance
Services will be performed within a 50-mile radius of 633 Indiana Avenue, NW, Washington, DC.
