Background: The Supai Health Station is an outpatient clinic located in the bottom of the Grand Canyon. It serves close to 600 patients. The contractor is required to provide regulated medical waste (RMW) and regular waste (RW) pickup and disposal services for the clinic. The waste will be transported by mule/horse to the Peach Springs Health Center for disposal. The contractor must comply with all applicable local, state, and federal regulations, codes, and laws, as well as The Joint Commission (TJC) Environment of Care standards.

Details: The tasks/services to be completed under the contract include:

- Pickup and disposal of regulated medical waste (RMW), which currently consists of red bag waste (including plastic 1 gallon sharps containers and the red bag waste). The red bag waste is collected daily and packed/wrapped in secured boxes. The average quantity is 4 boxes per month.
- Pickup and disposal of regular waste (RW), which consists of daily trash within clinic offices. The average quantity is 8 boxes per month.

The contractor must provide invoicing cost expressed in the amount of boxes of waste packed. Invoiced amount should reflect actual quantities. The pickup frequency for both RMW and RW is every two (2) weeks.

The contractor must provide 44-gallon plastic containers, Rubbermaid or equal, with lids suitable for transport of RMW and RW. Six (6) containers should be provided.

The hours of operation for both Supai Health Station and Peach Springs Health Center are from 8:00 am to 5:00 pm, Monday - Friday (excluding Federal Holidays). Any spills by the contractor must be cleaned up immediately and reported to the Contracting Officer's Representative (COR). The contractor must provide copies of manifests covering transportation of RMW from its pick-up point at the Supai Health Station to its final destination at the Peach Springs Health Center. The contractor must also provide a signed receipt for each shipment of RMW and RW to the COR. All activities that require prior approval of the COR must be coordinated with them. The contractor must comply with Supai Health Station and Peach Springs Health Center visitation policy, which may require obtaining a Contractor badge prior to beginning services. Travel expenses must comply with the GSA Federal Travel Regulation (FTR).

Period of Performance: The period of performance for the contract is from December 15, 2023, to December 14, 2028.

Place of Performance: The contract will be performed at the Supai Health Station located at 1 Main Street, Supai, AZ 86435, and the waste will be transported to the Peach Springs Health Center located at 943 Hualapai Way, Peach Springs, AZ 86434.
