Special Mode Transportation Services for Jesse Brown VA Medical Center Beneficiaries   7

Background
The Department of Veterans Affairs, Great Lakes Acquisition Center (GLAC) intends to issue Solicitation 36C25221R0084 for Special Mode Transportation Services to the beneficiaries of the Jesse Brown VA Medical Center. The VA intends to award a single Firm Fixed Price contract for one year and four option years. The procurement is set-aside based on an order of priority as established in 38 USC 8127 and solicitation issued as tiered evaluations with the following tiers: (1) SDVOSB concerns; (2) VOSB concerns; (3) small business concerns with HUBZone small business concerns and 8(a) participants having priority; and (4) large business concerns.

Work Details
The Contractor shall furnish the necessary personnel and equipment to provide for twenty-four (24) hour 7 (seven) day a week wheelchair van and sedan transportation services. Approximately 80% of the transports will occur entirely in the state of Illinois and 20% will have an origin or destination in the state of Indiana. The service will be provided from pick up location to the drop off location as directed by the VA.
The primary VA Medical facilities under this contract include Jesse Brown VA Medical Center and associated Community-Based Outpatient Clinics. Service will be from pick up location to the drop off location as directed by the VA.

Period of Performance
The VA intends to award a single Firm Fixed Price contract for one year and four option years.

Place of Performance
Chicago Area to include the Jesse Brown VA Medical Center and associated Community-Based Outpatient Clinics. Also includes other VA and non-VA facilities in the PWS.
