THIS REQUEST FOR QUOTATION AND SUBSEQUENT ORDER IS SET-ASIDE EXCLUSIVELY FOR ONLY THOSE AUTHORIZED FISC NORFOLK BLANKET PURCHASE AGREEMENT HOLDERS. QUOTES FROM OTHER THAN THESE VENDORS WILL NOT BE CONSIDERED. **QUOTES WITHOUT A NORFOLK FURNITURE BPA LISTED ON THEM WILL NOT BE CONSIDERED** LIKE ITEMS MAY BE CONSIDERED, PROVIDE SPECS FOR ITEMS. PLEASE INCLUDE FREIGHT CHARGES AS A SEPARATE LINE ITEM ON QUOTES. PLEASE PROVIDE SERVICE FEE CHARGES IF APPLICABLE. PURCHASE WILL BE MADE WITH GOVERNMENT PURCHASE CARD. 889 FORM MUST BE SUBMITTED WITH THE QUOTE. SPECS: See attached document for specifications. DELIVERY REQUIREMENTS/INSTRUCTIONS: Delivery trucks/installers must have DBIDS Gate access for all Navy Region NW sites in order to access the installations. Delivery notification must be made 48 hours in advance with driver's name, license number and state of issuance. Driver must be a US citizen and have proof of citizenship on his/her person (birth certificate with raised seal, passport, or enhanced driver's license). Deliveries that do not follow these instructions will be turned away at the gate. Delivery drivers must provide 48 hours or more notice prior to delivering items. Delivery must occur between 6 a.m. and 1 p.m. Monday through Friday. Please contact POC at the site to schedule delivery. DELIVERY INSTRUCTIONS ARE AS FOLLOWS: - Contractor responsible for installation and assembly of new furniture - Contractor responsible for removal and disposal of existing furniture - No loading dock at the facility. - Street parking is about 15 feet from entrance door.
