RFQ - Weekly dry ice delivery for Puget Sound VAHCS - Multiple delivery locations

Background
The Department of Veterans Affairs (VA) is issuing this Request for Quotation (RFQ) for the weekly delivery of dry ice to the Puget Sound VA Health Care System (VAHCS) in Seattle, Washington. This contract aims to support the Research and Development Lab Operations, which require a reliable supplier to deliver dry ice for use by over 40 research labs and clinical studies. The goal is to establish a contract with a base year and two option years to ensure consistent supply.

Work Details
The RFQ specifies the following requirements for the delivery of dry ice:
- Quantity: 70 slabs of 10 lbs each (totaling 700 lbs) delivered weekly on Mondays.
- Delivery Locations:
- 15 slabs (150 lbs) to Bldg. 8 Loading dock
- 15 slabs (150 lbs) to Bldg. 101 Loading area
- 40 slabs (400 lbs) to Bldg. 1 Loading area breezeway, placed into a blue dry ice bin.
- Deliveries are expected on the first day of the week after weekends, typically Mondays, with adjustments for federal holidays.
- The contractor must provide a completed and signed SF1449 form along with their quote, including company information such as UEI#, point of contact, email address, phone number, and physical address.

Period of Performance
Base Period: September 19, 2025 - September 18, 2026; Option Year 1: September 19, 2026 - September 18, 2027; Option Year 2: September 19, 2027 - September 18, 2028.

Place of Performance
Deliveries will be made to multiple locations within the Puget Sound VA Health Care System in Seattle, Washington.
