RFQ: Solicitation 36C25226Q0156-Optical Dispensing Services at the Jesse Brown VA Medical Center and CBOC   4

Background
The Department of Veterans Affairs (VA) is seeking to provide optical dispensing services for eligible veterans at the Jesse Brown VA Medical Center in Chicago, IL, and the Adam Benjamin Jr. VA Outpatient Clinic in Crown Point, IN.

The goal of this contract is to furnish eyeglasses to veterans requiring assistance, with fabrication performed by a VA-owned laboratory in Indianapolis, IN. The contractor will act as the dispensing optician.

Work Details
The contractor shall provide all labor, travel, materials, and necessary equipment to deliver optical dispensing services. Specific tasks include:

- Operating as the dispensing optician at both locations;
- Fitting, adjusting, and minor repairing of eyeglasses;
- Providing onsite tinting and UV/scratch coating of lenses;
- Entering into a retail concession agreement with the Veteran’s Canteen Service (VCS) to offer commercial optical services at discounted prices;
- Ensuring that upgrades or additional services are handled through the concessionaire agreement without recommendation from the contractor.

The contractor must also provide dispensing equipment such as pupilometry lensometry and adjusting tools, as well as maintain dispensing furniture in good condition at both locations.

Services will be performed at the Jesse Brown VA Medical Center located at 820 S. Damen Ave., Chicago, IL 60612 and the Adam Benjamin Jr. VA Outpatient Clinic located at 9301 Madison Street, Crown Point, IN 46307.
