Background
The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Geriatrics and Extended Care (GEC) Program Office has a requirement for conducting surveys at State Veterans Homes (SVHs). The goal of this contract is to ensure that SVHs maintain high-quality standards and comply with Title 38 Code of Federal Regulations (CFR) Part 51, which governs the per diem for nursing home, domiciliary, or adult day health care of veterans in state homes.

The contractor will conduct independent assessments of SVH compliance with these regulations through a structured survey process aligned with the Centers for Medicare and Medicaid Services (CMS) survey methodology.

Work Details
The contractor shall perform comprehensive surveys at SVHs, including Annual, For Cause, and Recognition surveys. Key tasks include:
1. Conducting surveys to assess compliance with applicable regulations and quality measures.
2. Utilizing qualified personnel with expertise in long-term care survey processes, including Registered Nurses, Social Workers, Dietitians, and Nursing Home Administrators.
3. Performing life safety and fire protection assessments by trained professionals familiar with National Fire Protection Association (NFPA) codes.
4. Completing all required VA forms during the survey process and ensuring they are accurate and submitted timely.
5. Providing detailed reports on findings, including areas of noncompliance and recommendations for improvement.
6. Maintaining primary source verification of personnel qualifications and training credentials.
7. Conducting unannounced surveys primarily but allowing for announced recognition surveys as needed.

Period of Performance
The contract will have a base period of 12 months with four optional 12-month extensions, allowing for a total potential performance period of up to 60 months.

Place of Performance
Surveys will be conducted at various State Veterans Homes across the United States, including all 50 states, territories, tribal lands, possessions of the United States, and the Commonwealth of Puerto Rico.
