Background
The Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Geriatrics and Extended Care (GEC) Program Office has a requirement for conducting surveys at VA owned and operated Community Living Centers (CLCs) and Spinal Cord Injury/Disorders (SCI/D) Long Term Care Centers. The goal of this contract is to ensure that these facilities maintain high quality standards and comply with federal regulations, specifically Title 42 Public Health Code Federal Regulations Part 483. The contractor will utilize the Centers for Medicare and Medicaid Services (CMS) survey process to assess the quality of care provided in these facilities.

Work Details
The contractor shall perform comprehensive surveys of CLCs and SCI/D Long Term Care Centers, which includes:
1. Conducting surveys to identify areas of concern regarding quality of care through direct observations and medical record reviews.
2. Assessing compliance with applicable regulations and standards.
3. Ensuring that quality of care and safety needs are met.
4. Educating staff on veteran-centered care practices.
5. Providing qualified personnel with expertise in long-term care surveying, including Registered Nurses, Social Workers, Dietitians, and specialists in life safety and fire protection.
6. Maintaining primary source verification of personnel credentials and experience.
