Description

This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format Revolutionary FAR Overhaul (RFO) in Federal Acquisition Regulation (FAR) subpart 12.202, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2025-06 (eff. 08/27/2025].

This solicitation is unrestricted.

FAR 52.212-2 Evaluation-Commercial Products and Commercial Services: In accordance with RFO FAR 12.201-1 and FAR 12.201-2 when appropriate, the contracting officer may (1) Insert the provision at 52.212-2, Evaluation-Commercial Products and Commercial Services, in solicitations for commercial products or commercial services.
  3. Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://sam.gov/.
  4. FAR 52.212-4, Contract Terms and Conditions Commercial Products and Commercial Services [NOV 2023]

To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

Background
The Michael E. Debakey VA Medical Center in Houston, TX is seeking Emergency Contingency Services for Hazardous and Non-Hazardous Medication Compounding. This requirement is crucial for providing treatments to Veterans diagnosed with specific medical conditions, with the facility serving an average of 50 patients per day.

The contract will be a firm-fixed price, Indefinite-Delivery-Indefinite-Quantity contract, ensuring compliance with FDA regulations and state laws.

Work Details
The contractor shall provide all hazardous and non-hazardous agents, IV tubing, closed system drug transfer devices, and any other necessary items. Compounding must be done at an FDA 503A compliant facility. Orders will be submitted via a secure network or fax and confirmed by VA staff.

Compounded orders must be delivered to the Michael E. Debakey VA Medical Center by 7:30 AM the day after orders are submitted. All drugs are to be supplied by the Contractor, who will invoice for billable units utilized for each patient. The contractor must provide certification reports for sterile compounding areas and action plans for any deficiencies found during inspections. Shipping costs are included in the order, with no additional charges for weekend deliveries.
