Work Details
1. The Contractor shall provide non-personal Pharmacy Hazardous Compounding Services for the Portland Veterans Health Care System.

2. The contract will be a five-year Indefinite Delivery Indefinite Quantity (IDIQ) Contract with multiple ordering periods.

3. All orders must be delivered no later than 8:00 am PST on the date of intended treatment, with a required accuracy rate of 97% or better.

4. The Contractor must comply with USP 797 and USP 800 regulations, preparing chemotherapy using aseptic techniques and delivering medications in appropriate containers with necessary refrigeration measures.

5. The Contractor will maintain documentation for certification of compounding areas and provide copies upon request.

6. In case of product shortages or delivery failures, the Contractor is responsible for providing replacements at no additional cost to the VA.
